Behavioral Health Care Coordinators (BHCCs) play a vital role in supporting members with behavioral health needs through care coordination, case management, member engagement, and resource navigation. BHCCs work closely with members, providers, community resources, and internal care teams to help ensure members receive timely access to appropriate services and support.

The primary responsibilities of a BHCC include conducting outreach, identifying member needs, coordinating care across providers and services, assisting with referrals, supporting treatment plan adherence, and helping members overcome barriers to care. BHCCs also provide education, encouragement, and ongoing support to promote member engagement and positive health outcomes.

Behavioral Health Care Coordinator (BHCC) Responsibilities

• Conduct patient outreach, engagement, and follow-up.
• Assess patient needs and identify barriers to care.
• Coordinate services between patients, providers, and community resources.
• Develop and monitor individualized care plans.
• Facilitate referrals and support appointment and treatment adherence.
• Support transitions of care, including hospital admissions and discharges.
• Maintain timely, accurate, and compliant documentation.
• Collaborate with interdisciplinary teams to improve patient outcomes.
• Escalate clinical, safety, or crisis concerns to appropriate licensed staff.
• Provide education and connect patients to behavioral health, medical, and social support resources.
• Utilize evidence-based interventions such as active listening, motivational interviewing (MI), behavioral activation, grounding techniques, coping skills reinforcement, safety planning, and problem-solving strategies to support patients experiencing crisis, anxiety, depression, or other behavioral health challenges.

Behavioral Health Care Coordinators do not provide therapy, counseling, psychotherapy, diagnosis, or other licensed clinical treatment services. Their primary role is care coordination, case management, patient support, and resource navigation.

Requirements:

• Master's Degree in any field; or
• Bachelor's Degree in Psychology, Social Work, Human Services, Public Health, Behavioral Health, or a related field; or
• Bachelor's Degree in any field with at least one (1) year of relevant experience in behavioral health, healthcare, care coordination, case management, or human services; or
• High School Diploma/GED with four (4) or more years of relevant experience in behavioral health, healthcare, care coordination, case management, or human services.

Additional Qualifications:

• Experience in behavioral health, care coordination, case management, healthcare, or a related human services setting.
• Knowledge of behavioral health conditions, community resources, and social determinants of health.
• Ability to work effectively as part of a multidisciplinary team.
• Strong communication, engagement, and relationship-building skills.
• Ability to maintain professional boundaries, confidentiality, and ethical standards.
• Strong organizational, documentation, and time-management skills.
• Proficiency in electronic health records (EHRs) and care management platforms.
• Ability to utilize evidence-based engagement techniques such as motivational interviewing, active listening, behavioral activation, grounding techniques, safety planning, and problem-solving strategies.
• Commitment to ongoing professional growth and development.

Preferred Qualifications:

• Experience working with Medicaid, Medicare, or managed care populations.
• BHT certification (if applicable).
• Bilingual or multilingual capabilities.
• Experience supporting individuals with behavioral health, substance use, or complex care needs.
